Ready to start the self-determination program?

For participants interested in starting the Self-Determination Program (SDP), email intake@engagedservices.org.

Please include the Name of Participant, confirmation that you have completed the required 2-hour training course* (or have it on your schedule), and informed your ACRC Service Coordinator that you are pursuing SDP.

*Reminder that if the participant is over 18 years old and is NOT conserved, the certificate MUST be in their name.
